Anyone may make a CARE Referral about a Wake Forest University community member (student, faculty, or staff). You may do so using the referral form, emailing CARE@nullwfu.edu, or contacting a CARE Team member office or liaison office and stating that you wish to make a referral. It’s helpful, although not required, that you identify yourself when communicating your concern. All information shared with a member of the CARE Team is kept as private as possible and shared only with essential, need-to-know parties, but it is important to note that it is not strictly confidential. Accordingly, you may submit a CARE Referral anonymously if you so choose.
Helpful Information When Making a Referral
In order for the CARE Team to best meet the needs of the individual of concern and the community, the Team needs the most accurate information possible. If you are making a referral, or reporting situations or behaviors of concern, please be prepared to share the following relevant information:
- Date/time of incident(s)
- Name, age, relationship to university, and current location of the community member you’re referring
- Description of concerns, worrying behaviors, or acts committed
- Names of witnesses
- Location where incident occurred
- Related documentation, such as email, text, Facebook, blogs, or other electronic communications, photos or videos
Upon receiving a referral, Team members consult to ensure thorough assessment and follow up.
Though some of its members represent offices which are involved in crisis response, the CARE Team is not tasked with responding to emergencies.
